Survivors include his wife, Jessica; daughter, Morgan; former spouse, Jennifer; three sons, Andrew, Jacob and Matthew; mother, Sharon Amaya; father, Richard Wertz; three siblings, Danielle, Paul and Stefanie.
Private services will be held.
Hull & Hull Funeral Directors are in charge of arrangements.
