David Michael Wertz, age 42, of Grants Pass, died Tuesday, January 2, 2018 at his home.

Survivors include his wife, Jessica; daughter, Morgan; former spouse, Jennifer; three sons, Andrew, Jacob and Matthew; mother, Sharon Amaya; father, Richard Wertz; three siblings, Danielle, Paul and Stefanie.

Private services will be held.
